A fungal ear infection, also known as otomycosis, is an infection of the ear canal caused by fungi. It can sometimes be misdiagnosed as an outer ear infection (otitis externa) due to similar symptoms such as itching, discharge, and discomfort. In general, fungal ear infections are less common than bacterial ear infections.

Common symptoms of otomycosis include:

  • Itching or irritation in the ear canal
  • Thick yellow discharge

What are the Causes and Risk Factors of a Fungal Ear Infection?

Fungal ear infections typically occur due to an overgrowth of fungi such as Aspergillus and Candida in the ear canal. Some groups of people are more likely to develop fungus in the ear than others because of certain factors including:

 

  • Regular water exposure: Regular exposure to water can lead to a condition known as โ€˜swimmerโ€™s earโ€™, where excess moisture in the ear canal reduces the protective function of earwax. This, in turn, increases the risk of a fungal ear infection.
  • Diabetes: Diabetes can impair immune function and blood circulation, making it harder for the body to fight infections. As such, individuals with diabetes are at greater risk of developing otomycosis.
  • Compromised immune system: Individuals with conditions such as HIV/AIDS, cancer or those undergoing immunotherapy treatments such as chemotherapy may find it difficult to combat infections.
  • Eczema or dermatitis: Skin conditions that cause inflammation or irritation in the ear canal can compromise the skinโ€™s barrier, increasing the individualโ€™s susceptibility to otomycosis.
  • Humid climates: Warm and humid environments tend to allow fungi to thrive. Thus, those living in humid or tropical climates may be more at risk of developing fungal ear infections.
  • Frequent ear cleaning: Earwax serves as a natural protective barrier for the ear canal. It has antibacterial and antifungal properties that help inhibit the growth of harmful bacteria and fungi. Individuals who use cotton swabs or other tools to clean their ears may end up removing too much earwax, increasing their vulnerability to ear infections.

  • Avoid digging your ears: Allow your earโ€™s natural cleaning mechanism to clear out earwax rather than cleaning your ears on your own. Retaining a small amount of earwax helps prevent otomycosis.
  • Avoid inserting objects into your ear canal: Refrain from inserting cotton swabs or any other objects into your ears, including your own fingers as they may be in contact with fungal spores that are difficult to spot with the naked eye.
  • Keep ears dry: Prevent water from entering the ear canal during water activities by wearing swim plugs or a swimming cap, especially if the water body is poorly maintained or poor hygiene is suspected. Additionally, dry your ears thoroughly after the water activity.
  • Avoid scratching your ear: Be careful not to scratch the skin around or inside your ears.

If otomycosis is left untreated, it can lead to ongoing discomfort, such as itching, pain and chronic discharge, along with temporary hearing loss.

In severe cases, untreated ear infections may spread to the temporal bone of the skull or cause persistent inflammation in nearby tissues. This potentially results in more serious complications, for example meningitis, mastoiditis and fungal osteomyelitis.
